Before Your Appointment
Make sure that we have your current insurance information and a referral, if required by your insurance, before the day of your appointment. If you are an injury patient (injured at work and/or auto accident) please provide all your detailed information to the staff before your appointment to expedite the process.
Make sure that you are able to provide us with contact information for your primary care provider and your referring provider. We are part of your health care team and want you to have the best possible interactive care with your primary care physician. This is how we are able to provide you the most comprehensive plan of care.
Arrive 15 to 30 minutes before your appointment to fill out your new patient packet, which includes a patient questionnaire, HIPAA acknowledgement, medical release and registration forms. We realize that we request a lot of information from you; however, it is necessary to provide the best quality care that you deserve.To expedite the process, you can also print these at home, and bring the completed forms with you to your visit.
It is important to provide information about all medications with their strength and dosage. Please include regularly prescribed medications, those you take on an “as needed” basis, over the counter medications and preparations, vitamins, herbals, Chinese, homeopathic, medicated creams, ointments, eye drops, etc., as they may need to be stopped before your injection or they might interact with medicines prescribed from the pain clinic.
Provide copies of all pertinent lab and imaging reports. If these were done by someone other than your referring provider, you may need to request them from the ordering provider prior to your appointment.
On the day of your appointment
The first visit is usually a consultation. The physician will take your history, examine you and then devise a treatment plan with you.
Arrive 15 minutes prior to your scheduled appointment. This allows time for you to complete the paperwork process with our office staff and update your current information.
During Your Consultation
Your weight and vital signs will be taken.
The physician will talk to you about your medical history, perform a physical exam and talk to you about your goals.
The physician may order additional tests, such as blood tests, special imaging and other diagnostics if indicated to provide an accurate diagnosis and ensure your safety.
Our staff is very experienced and helpful in getting pre-certifications for your diagnostic imaging (MRI scans, CT scans, etc.) and will help you in making appointments, choosing a facility and obtaining the authorizations. In order for us to obtain the authorization, you must schedule the appointment and then notify our staff so we are aware of the location and date of your procedure.
If your insurance does not require any pre-authorization, and you do not have complicated health issues, you may be eligible to have a procedure on the day of your consultation. However, in most cases only a consultation will be performed.
